Ability to participate, declaration
I confirm my child is physically fit and healthy and I will undertake to advise you of any change. I consider him/her capable of taking part in gymnastics. I have completed the section on medical details and give consent that in the event of any illness/accident any necessary treatment can be administered. When accidents occur, they will be recorded and reported to the parent / responsible adult collecting the child at the end of the session.
Code of conduct for Participants
We are fully committed to safeguarding and promoting the well-being of all our members. The club believes it is important that members, coaches, administrators and parents associated with the club should, at all times, show respect and understanding for the safety and welfare of others. Therefore, members are encouraged to be open at all times and share any concerns or complaints that they may have about any aspect of the club with the clubs welfare team
As a member of Nottingham city gymnastics club you are expected to abide by the following club rules:
-All members must participate within the rules and respect coaches, judges and their decisions
-All members must respect opponents and fellow club members
-Members must wear suitable attire for training and events as agreed with the coach. Keep all long hair tied back and remove all body jewellery
-Members should treat all equipment with respect
-Members must inform the head coach of any injuries or illness they may have before the warm-up begins
-Members should not eat or chew gum during a session
-Members should remain with coaches at the end of a session until collected by their parent or guardian
I confirm that I have read through the Participant’s Code of Conduct with my child and they understand and agree to abide by the rules.
In signing up to attend the club I declare that I am aware of the element of risk involved and while I accept that the coaches and event personnel will take precautions to prevent accidents, I understand that they may not be held responsible for loss, damage, or injury to my child.
I am aware that if I wish to cancel this subscription, I must notify the club in writing a minimum of 14 days before the next payment is due and understand that fees are paid across 12 months for sessions running during term time.
Fees and payments:
Subscription payments are paid over 12 months to cover sessions during term time. Where payment isn't made, the place at the club will be cancelled and offered to the next person on the list. We want to encourage participation, if you're having trouble keeping up with the payment please speak to us to discuss options.
To cancel a subscription and email must be sent to the club a minimum of 14 days before the next subscription date.
Your subscription pays for your space in the class regardless to whether your gymnasts attends or not.
Please note, for sessions that are cancelled for circumstances out of our control, no refund will be given.
Photo Consent
At Nottingham City Gymnastics, we value the privacy and safety of all our gymnasts. In order to share the achievements and experiences of our gymnasts with the broader community, we sometimes take photographs or videos during training sessions, events, or performances. These photos may be used on our social media platforms, website, and promotional materials.
By answering yes to photo consent, you consent to the use of photographs or videos of your child (or yourself, if you are the gymnast) taken by Nottingham City Gymnastics under the following terms:
1. Consent for Photography and Videography
I give permission for Nottingham City Gymnastics to take photographs and/or videos of my child (or myself) during gymnastics activities or events
2. Usage of Photos and Videos
I understand that the photos and videos may be used for the following purposes:
- Social Media: Posting on platforms such as Facebook, Instagram, TikTok, etc.
- Website: Featuring on the club's official website.
- Promotional Materials: Including flyers, posters, brochures, and other marketing materials.
3. Duration of Use
I understand that these images may be used indefinitely for the purposes outlined above unless I request to revoke this consent in writing.
4. No Compensation
I understand that no compensation will be provided for the use of these photographs or videos.
5. Privacy and Confidentiality
Nottingham City Gymnastics will ensure that the privacy of all gymnasts is respected. We will not include any personal information (such as names or addresses) in any posts, unless explicitly agreed upon.
6. Opting Out
I understand that participation in this photo consent is optional. If I do not wish my child (or myself) to be included in any photographs or videos, I may opt out at any time by changing my photo permissions on the class4kids account.
7. Liability
I acknowledge that Nottingham City Gymnastics will take reasonable precautions to ensure the safety of my child (or myself) during photography and videography sessions. I hereby release Nottingham City Gymnastics and its representatives from any liability related to the use of the photographs or videos taken.
8. Confirmation
By answering yes to the photo contest registration question, I confirm that I have read and understood the terms and conditions outlined above. I agree to allow Nottingham City Gymnastics to take and use photographs and videos as described.
Collection from sessions:
All participants must be collected from sessions by a responsible adult. Gymnasts will only be permitted to leave unaccompanied if the permission to leave alone survey has been completed.
